I/We hereby make an application in accordance with provisions of sub-clause (ii) of clause 4 of the Plant Quarantine (Regulation of Import into India) Order, 2003 issued under sub-section (1) of Section 3 of the Destructive Insects and Pests Act, 1914 (2 of 1914), for permission to import soil/peat or sphagnum moss or other growing media of plant origin as detailed below:

1.	Description of growing media intend to import (soil/peat or sphagnum-moss etc.)
2.	Quantity to be imported
3.	Country of origin & foreign port of shipment
4.	Mode of package and No. of Packages
5.	Means of conveyance
6.	Point of Entry
7.	Name & Address of the Importer
8.	Name & Address of the exporter
9.	Specific purpose of import
10.	Approximate date of import

I/We hereby undertake to pay to an officer duly authorized by the Plant Protection Adviser the prescribed fees towards inspection, fumigation or treatment of the above consignment and abide by the instructions/guidelines issued by him.
